Print a copy of the Parental Consent Form.

Complete the appropriate form details. Make sure to sign the form.

Bring the form to an event or scan the form and send it by e-mail to

Details
Changes in insurance rules make it necessary for us to expand our requirements for parental consent.
We now need to have a copy of the parental consent form on file.
In addition, some children will only be allowed to participate when accompanied by an authorized adult.
We hope these changes to our parental consent policy don't cause difficulties for anyone.

Children under 18:

Must have a completed parental consent form in order to participate.
The form includes express consent for the under-18 participant to complete the
event unaccompanied if so desired.

Must have a completed parental consent form in order to participate.
The form includes express consent for the under-14 participant to complete
the event unaccompanied if so desired.

Must be accompanied to and from the event site by an adult.

Children under 10:

Must have a completed parental consent form in order to participate.

Must be accompanied to event site by an adult.

Must be accompanied by an authorized adult to and from the event and at
all times during the course. The name of the authorized adult must be
included on the parental consent form.

The parental consent form applies to events for the entire season.
Each participant under age 18 will only need to have the form completed once for the entire season.
We will keep a copy of the form on file.

The need for a parental consent form is determined by the age of the child on the day
of the event.

For purposes of the GPHXO parental consent form, and "adult" is defined as anyone 18-years old or older.

All children under 14-years old must be accompanied to and from the event
by an adult.

We require an authorized adult to accompany children under 10 to and from the event
and at all times on the course. The name of the authorized adult must be included on the
parental consent form.

Regardless of the information on the parental consent form, we recommend that all children under age
18 participate in groups of two or more individuals.
